Sign-up forms for personalized passes

Any pass with {variables} can now have its own sign-up form, not just loyalty passes. Every variable on the pass becomes a field, labelled from its name, and you can add questions that aren't on the pass too.

People are recognised by their email or phone number. If someone signs up twice, nothing is duplicated: their link is sent to the email or phone on file. People you added ahead of time in Dynamic data are matched to their sign-up the same way.
